APPLICATION
For all non-residential users who are served through a single meter
and for three phase residential or non-residential use taken through
a single meter at the Utility's standard secondary voltage, up to
a nominal limit of 5 kva demand. Governed in more detail by Standard
Rules and Regulations of the City.

ELECTRIC RATE SCHEDULE - GENERAL SERVICE MEDIUM
APPLICATION
For any non-residential use through a single meter at the Utility's
standard voltages up to a nominal limit of 90 kva demand. The customer
is responsible for furnishing, installing and maintaining his transformers,
etc. At the discretion of the Utility, customer with less than 55
kva demand may be metered at the secondary voltage and customers
with 55 kva demand and above may be metered at the primary voltage.

or by suitable demand meter continuously connected to indicate the

ELECTRIC RATE SCHEDULE - INDUSTRIAL
APPLICATION
Available to industrial customers and municipal corporations with total
demands of 800 kva or over which may be metered on primary side of
customer's equipment. An industrial customer is defined as one who
assembles, manufacturers or processes a product for resale from one
or several parts. Customer may not transfer or sell power to another
party unless the customer is a municipal corporation. Governed in
more detail by Standard Rules and Regulations of the City.
MINIMUM APPLICATION CHARGE
$3.20 per kva demand but not less than 800 kva demand.
SERVICE CHARGE
None inside the corporate limits. This rate should be applied outside
the corporate limits by addition of $2.73 per month.
CONTRACT TERM
Application accepted for supply under this tariff will be contracted
for a mutually agreeable period, with automatic self-renewal provisions
until the next December 31 and thereafter until either party shall
give at least sixty (60) days written notice to the other of its intention
to discontinue at the end of any calendar year.

EXHIBIT 1 - POWER COST ADJUSTMENT
A Power Cost Adjustment (PCA) factor will be included in each bill
for service rendered. The adjustment shall be the product of the total
kilowatt-hours times the PCA.
The PCA will be adjusted quarterly (or less often as determined by
the Public Utilities Board) based on increases/decreases in fuel cost,
ash disposal fees, environmental fees and purchase power costs less
a portion of off-system sales divided by the billed kilowatt hours
during the period. The PCA will include
adjustments for accelerated debt service and a credit to smooth our
purchase power "spikes".
The current PCA is $0.0184137/ kWh,
effective December 15, 2009 billing.

EXHIBIT 2 - KILOWATT HOUR TAX
The kWh tax is based on actual kWh delivered to customers (including
the municipality) on the distribution system, excluding federal facilities.
If no meter is used, the usage will be estimated. The tax applies to
all kWh distributed, even if there was no charge for service. The tax
is based on the number of kWh's used in a given month by a customer.

The above rates are based on a 30-day billing cycle. If your billing
cycle is 30 days exactly, the above referenced rates will be used.
If the billing cycle is less than or more than 30 days, the daily calculation
will be used.
The daily calculation will be determined as follows:
Determine the number of kWh used during the billing cycle;
Divide that number by the number of days in the billing cycle (to come
up with the daily average usage). Retain at least 5 decimal places
(x.xxxxx) when calculating the daily average usage.
Take the daily average usage and (for each block, round the tax amount
to the nearest whole cent (x.xx))
— Multiply $0.00465 for the first 67 kWh distributed;
— Multiply $0.00419 for the next 68 to 400 kWh distributed;
— Multiply $0.00363 for the remaining kWh distributed.
The tax is the sum of the above multiplied by the number of days in
the billing cycle.
All customers within the city limits of Orrville receive a credit on their monthly utility invoice of the above mentioned State KWH tax through their monthly utility invoice of the State KWH tax through the "Community Tax Credit", see Exhibit 3. Those customers outside the city limits who are charged this tax do not receive the credit but their tax dollars are sent to the State by mandate.

The community tax credit is applicable only to customers whose service
is within the city limits of Orrville. The amount of the credit will
be adjusted annually (or less often as determined by the Public Utilities

If new pole is required:
1. The pole may be purchased and installed for a contract price of
$225.
2. The pole may be rented at a contract price of $3.60 per month along
with an installation fee of $46.00 The customer will also have the
option to purchase the light with the following conditions:
1. The light will be sold at the City's current market
price. (Mercury vapor lights will eventually be phased out. All new
lights will be
the high pressure sodium type.)
2. Installation cost for the light will be $41.00.
3. The light must be metered.
4. The customer will be responsible for connecting
the light.
5. The customer will be responsible for maintaining
their light or may choose to pay a monthly charge of $2.30 for the
City to maintain it.
